Our community health workers, also known as CHWs, are vital to the work we are able to do at CCI and essential in providing holistic client care. Mental Health CHWs

Our mental health CHW’s are able to provide a large array of services including emotional support, crisis response support, education on coping skills, and more. They are also able to assist in providing case management services and additional interal and external resourcs and referrals.

Early Childhood CHWs

Our Early Childhood CHWs focus on bring literacy resources and education into our rural communities. Throughtout the month, they host free story time events at libraries throughout Lyon County. They hope that through these events, family libraries grow from free books that each child takes home and families have a special moment to bond and share in fun activities. General CHWs

Our General CHWs are able to provide assistance on a much broader scale. They are able to assist with assessing needs and helping client’s prioritize their approach to overall wellness. Additionally they can help locate and access other resources in the community to address needs. They are also able to assist clients through direct services such as employment assistance. General CHWs are able to assist in resume building, job searching, interview preparation, and provide other employment education. For those who are interested and qualify, they are able to provide full case management services to individuals who enroll into the Displaced Homemaker (DH) Program.

The DH program is intended to assist people who were previously supported financially by an individual who is no longer able to provide that financial support. The goal of the DH program is to help the client reach long-term stability and self-efficiency through education and employment. For more information on the DH program, please complete the form.
